A major is a formalized curricular sequence taken by a student within his/her major area of study. Majors must be approved by AAC, Plenary, AVP, Board of Trustees, and the NYSED before they can be printed in the Catalog or be offered to students.

A minor is a formalized curricular sequence taken by a student outside his/her major area of study. Minors must be approved by AAC, Plenary, AVP, and Board of Trustees before they can be printed in the Catalog or be offered to students.

A certificate is a credential issued by an institution in recognition of the completion of a curriculum other than one leading to a degree approved by the AAC, Plenary, AVP, Board of Trustees, and the NYSED that is sufficient to master a well-defined body of knowledge or technical expertise; an advanced certificate is a credential beyond the baccalaureate.

A concentration or track is a formalized curricular sequence approved by the AAC, Plenary (or Petition) and the AVP to achieve a specific goal within a major. These goals may be externally controlled in order to achieve certification or accreditation or be internally controlled in order to define more closely a curricular package or make an area of study more visible and salable.


PROGRAM REVISION FORMPDF

All program revisions are approved by the AAC and AVP; some may also require petition action, Plenary vote, Board of Trustees, and/or NYSED approval. Use the Routine Course Change form for revisions to existing courses; do not include individual course revisions on this form. Program revisions are NOT retroactive to preceding semester or Catalog; however, program revisions that will impact students using earlier Catalogs for their degree requirements must be noted (see #11 below).

This form should be used to request the following types of curriculum revisions:

  • Revising the name of a degree program, major, minor, concentration or track
  • Revising the admission criteria for a major/minor/concentration/track
  • Revising the criteria for progression in a major/minor/concentration/track
  • Revising the GPA requirements for a major/minor/concentration/track
  • Revising the number of hours required for a major or one of its minors/concentrations/tracks
  • Adding an option or concentration within a baccalaureate program
  • Changing the focus, design, title, location, format, calendar, or liberal arts content of a baccalaureate program
  • Revising an undergraduate or graduate level program
  • Revising a program leading to teacher certification
  • Revising a certificate, diploma, or advanced certificate program.

NOTE: Revisions to specific courses must be submitted
on a separate Routine Change Form


REQUEST FOR DISCONTINUATION OF A MAJOR, MINOR, CERTIFICATE, PROGRAM, CONCENTRATION, OR TRACKPDF

Use this form for the discontinuation of a major, minor, certificate, program, concentration, or track. (You may fill out this form on your computer. First, save the PDF form to your desktop or folder and then follow the Typewriter Tool instructions at the top of the form. You must have Adobe Reader 7.0.5 or later to use this function.)

A discontinuation of a major, minor, certificate, program, concentration, or track requires the same procedure as that outlined for planning and establishing a major, minor, certificate, program, concentration, or tracks. After approval by the department, the proposal must be approved by the Dean. After those signatures are affixed, the proposal must be approved by the Academic Affairs Committee. Finally, the form, accompanied by a cover memo, should be forwarded to the AVP, Board of Trustees, and the NYSED . Note that curricular changes are not effective until they appear in the Catalog.

If you are originating this form, you must provide the Curriculum Committee (CC) or the Academic Affairs Committee (AAC) evidence that you have consulted with other interested units concerning the objectives and content of the proposed new major, minor, certificate, program, concentration, track, or course. Please attach a copy of this cover sheet to a copy of the appropriate form (prepared for the CC or the AAC) and forward it to the interested units. After receiving feedback, attach a copy of this cover sheet(s) to the material to be forwarded to the CC.
